Jenn Bistrack
Jenn Bistrack is the Post-Acute Program Manager with UM Baltimore Washington Medical Center. She collaborates closely with acute, post-acute, and outpatient partners to extend best practices across the care continuum. Jenn started her career in public health, gaining experience in community health and health policy at local and state levels. She developed an interest in transitions of care as a community health nurse, working with complex patients after hospital discharge to help them stabilize in the community.

Jennifer Bierhup
Jennifer Bierhup is a wound care innovator and educator with nearly two decades of experience, transforming clinical practice in both acute and post-acute care settings. She holds advanced certifications including Wound Care Certification (WCC) from the National Alliance of Wound Care and Ostomy (NAWCO) and Case Management Certification (CCM) from the Commission for Case Management Certification. She is the creator of multiple nationally adopted frameworks that have elevated clinical standards across the industry.

Mary Martin
Mary Martin is a board-certified wound specialist through the American Board of Wound Management and a nursing leader with nearly two decades of experience across diverse healthcare settings. As a Skin Integrity Coordinator, she implemented strategies that reduced facility-acquired pressure injuries to zero in a skilled nursing facility and introduced a mobile wound tracking system to enhance care coordination. She has held leadership positions including Director of Nursing and Senior Skin Health team lead, overseeing 16 facilities. As Clinical Quality Lead at Tissue Health Plus, Mary now drives innovation in mobile healthcare solutions that support evidence-based, high-quality patient care, and is pursuing her master's in Nurse Leadership.
