Who is the checklist for?
It supports three groups: wound providers and wound provider groups building or improving a program, healthcare leaders at any post-acute site of service who are selecting a wound provider or group, and patients or responsible parties who want a reference guide when choosing one.
Which sections do I complete?
Complete Section I, Core Standards, in full. Then complete only the setting-specific add-on for your care setting: Skilled Nursing Facility (SNF), Home Health Agency (HHA), or Assisted Living / Independent Living (ALF/IL).
How is the checklist scored?
Each criterion is scored from 0 to 3: 0 is Absent, 1 is Emerging, 2 is Established, and 3 is Best-in-Class. Critical items are flagged with a star to signal elevated regulatory or clinical risk that needs priority follow-up.
